ECFMG 2027 Pathways Deadlines: OET by December 2026 and What Else to File

ECFMG opened the 2027 Pathways for ECFMG Certification on 15 May 2026, together with the online application for Pathways Revalidation. The Pathways are how international medical graduates meet the clinical and communication skills requirements for certification, and anyone planning to enter the March 2027 Main Residency Match must have them in hand by the NRMP's deadlines. With October here, the item most likely to sink a Pakistani applicant is not the Pathway itself but the English test behind it.

Who needs to apply

ECFMG's announcement names two groups: IMGs who still need to meet the clinical and communication skills requirements for certification, and IMGs who need to revalidate a previously accepted Pathway that has expired or is expiring. Applicants with a currently valid passing Step 2 CS result, or who are barred or whose certificate is suspended, are ineligible. Applicants who do not meet the criteria for Pathways 1 to 5 apply to Pathway 6.

The six Pathways in one line each

  1. Pathway 1: holders of a licence to practise medicine without supervision, dated on or after 1 January 2022.
  2. Pathway 2: graduates of schools that administer an OSCE required for licensure.
  3. Pathway 3: graduates of schools accredited by an agency recognised by the World Federation for Medical Education.
  4. Pathway 4: graduates of schools accredited by an agency with an NCFMEA comparability determination.
  5. Pathway 5: graduates of joint-degree programmes with an LCME-accredited US school.
  6. Pathway 6: six in-person outpatient encounters, each evaluated by a licensed physician using the Mini-CEX.

Pathways 2 to 5 require a medical school graduation date on or after 1 January 2024 under the 2027 rules. Pakistani graduates typically look first at Pathway 3 (PM&DC's WFME recognition status is the question to check) and, failing that, Pathway 6.

The deadlines that matter now

  • OET Medicine: ECFMG's FAQ says you must take OET Medicine "on or before the last scheduled test date in December 2026", and advises testing as early as possible so that a poor result can be rebooked. The minimum scores are 350 in Listening, Reading and Speaking and 300 in Writing, achieved in one sitting.
  • Fee: the Pathways application fee is $945 and is non-refundable.
  • Match timing: to be eligible for the Match, every component of a Pathway must be successfully completed by the NRMP's deadline, together with valid passing results on Step 1 and Step 2 CK. ECFMG points to the NRMP Rank Order List certification deadline as the date that governs when OET results must be in.
  • Pathway 1 verification: licence verification must reach ECFMG by 31 January 2027 for Match 2027, according to the published requirements.

Certificate expiry

Under the 2027 rules, your ECFMG Certificate expires when your Pathway expires; secondary sources place the expiry of 2027 Pathways certificates at 31 December 2029 unless made permanent by twelve months of ACGME-accredited training or an unrestricted US licence. Check ECFMG's expiration page for the official wording before relying on a date.

What this means for you

  1. Book OET Medicine now for a date that leaves room for a retake before the last December sitting. Results take days for the computer and at-home modes and about twelve days for paper.
  2. Decide your Pathway and gather its documents; Pathway 6 needs physician evaluators and scheduled encounters, which take weeks to arrange.
  3. Keep Step 1 and Step 2 CK results valid and your FSMB and ECFMG accounts in order; FSMB handles USMLE services for IMGs since January 2026.

FAME's OET and USMLE Step 2 resources are built around this calendar, and the platform's OET practice sets target the Medicine sub-tests ECFMG scores.
