OET for Doctors and Nurses: Four Sub-Tests, Twelve Professions and Three Ways to Sit It

The Occupational English Test (OET) is an English-language examination written for healthcare. Unlike general tests, its tasks are set in clinical contexts: a consultation to listen to, a referral letter to write, a role-play with a patient. It is accepted by regulators including the UK's GMC and NMC, ECFMG in the United States and the Australian health boards, which is why it appears in so many of FAME's candidates' plans. This article explains the test as the OET organisation describes it and helps you choose how to sit it.

Twelve professions

OET is "tailored to 12 healthcare professions": medicine, nursing, dentistry, dietetics, pharmacy, physiotherapy, podiatry, radiography, occupational therapy, veterinary science, optometry and speech pathology. The Listening and Reading sub-tests are common to all professions; Writing and Speaking are profession-specific, so a doctor and a nurse sit different Writing and Speaking tasks.

The four sub-tests

  • Listening (approximately 40 minutes): consultation extracts, presentations and workplace dialogues, with note-completion and comprehension tasks.
  • Reading: healthcare texts in three parts, from quick-reference skimming to careful reading of longer passages.
  • Writing: a profession-specific task, typically a referral, transfer or discharge letter written from case notes.
  • Speaking: role-plays with an interlocutor in which the candidate plays the clinician and the interlocutor the patient or carer.

Results give an overall score and individual scores for each sub-test. Regulators set their own minimums per sub-test; ECFMG, for example, requires 350 in Listening, Reading and Speaking and 300 in Writing in one sitting, and the GMC publishes its own required grades. Check the regulator's page, because the thresholds differ.

Three delivery modes

  1. OET Test on Paper at a secure test centre, with results typically within about twelve days.
  2. OET Test on Computer at a test centre, with results typically within about six days.
  3. OET@Home, taken remotely on your own computer under online proctoring, also with results in about six days.

Choosing between them is practical: computer and at-home modes return results faster, which matters when a deadline such as ECFMG's December 2026 OET cut-off is near; the at-home mode has equipment, room and connection requirements that must be met exactly; paper suits candidates who prefer handwriting the Writing task.

Preparing well

  • Writing is the usual weak point for doctors. The task rewards selecting what the reader needs from the case notes, not reproducing them. Practise against the published criteria with feedback.
  • Speaking rewards clinical communication: structure, empathy, checking understanding and clear explanation, which house officers can rehearse daily.
  • Listening and Reading need timed practice with healthcare texts, not general English.
  • Book with a retake in mind. If your regulator requires all minimums in one sitting, a single weak sub-test means a full retake; leave time.

What this means for you

Treat OET as a healthcare examination with a language component, choose the delivery mode by your deadline and circumstances, and give Writing the time it needs. FAME's OET package focuses on the medicine and nursing sub-tests with Writing practice marked to the criteria.

Two questions candidates ask

Do a doctor and a nurse sit the same test? Partly. Listening and Reading are common to all twelve professions; Writing and Speaking are profession-specific, so the letter task and the role-plays differ for medicine and nursing. Prepare with materials for your own profession.

How long is a result valid? Validity is set by the regulator you are applying to, not by OET, and the periods differ between bodies. Check the regulator's English-evidence page and time the test so the result is still valid at the point of application or registration.
