The Occupational English Test (OET) is the English examination ECFMG accepts for the communication skills requirement of its certification Pathways. Pakistani applicants often treat it as the easy part of the application and leave it to the end. The 2027 Pathways rules show why that is risky: the test must be taken by the last scheduled date in December 2026, the required scores must all come from one sitting, and results must reach ECFMG in time for the Match deadlines.
What ECFMG requires
- The Medicine version of OET, not a general English test.
- Minimum scores: 350 in Listening, Reading and Speaking, and 300 in Writing.
- One test administration: all four minimums must be achieved in the same sitting. You cannot combine a Speaking score from March with a Writing score from June.
- Timing: for the 2027 Pathways, take the test on or before the last scheduled OET test date in December 2026; ECFMG advises testing as early as possible so that an unsatisfactory result can be rebooked.
- Delivery of results: OET sends results to ECFMG directly; ECFMG's guidance ties the final timing to the NRMP Rank Order List certification deadline.
The test in brief
OET is tailored to twelve healthcare professions, medicine among them, and has four sub-tests: Listening, Reading, Writing (a profession-specific letter-writing task) and Speaking (role-plays with an interlocutor). It can be taken on paper at a test centre, on computer at a centre, or at home (OET@Home). Results are typically available within about six days for the computer and at-home modes and about twelve days for the paper test. On the reporting scale, 350 corresponds to grade B and 300 to grade C+, which is why ECFMG's requirement is often summarised as "B in three sub-tests and C+ in Writing".
Why Writing is the usual stumbling block
Pakistani doctors generally meet the Listening and Reading thresholds with modest preparation, and Speaking rewards clinical communication habits most house officers already have. Writing is different: the task is a referral or discharge letter built from case notes, marked on criteria such as purpose, content selection, conciseness, genre and style, organisation and language. Doctors who write detailed clinical notes tend to over-include; the task rewards selection. The lower threshold (300) reflects the difficulty, not leniency.
A timeline that works for Match 2027
- October 2026: book a test date in late October or November. Choose the delivery mode you can practise for; the at-home mode has technical requirements.
- Four to six weeks before: daily practice of letter-writing against the criteria, with feedback; weekly Listening and Reading timed sets; two Speaking role-plays a week.
- Test day: keep the one-sitting rule in mind; every sub-test counts.
- Results: if one sub-test falls short, rebook immediately; the December cut-off leaves exactly one retake window for most candidates.
- After results: confirm in your ECFMG application that the scores have arrived.
What this means for you
Treat OET as a scheduled examination with a hard deadline, not as paperwork. The one-sitting rule means a single weak sub-test costs a full retake, and the December cut-off means there may be no second chance for Match 2027. FAME's OET package focuses on the Medicine sub-tests ECFMG scores, with Writing practice marked against the published criteria.
Two questions candidates ask
Can I combine sub-test scores from two sittings? No. ECFMG requires the four minimums in a single test administration. A sitting with three strong scores and one weak one has to be repeated in full.
Which delivery mode returns results fastest? The computer-based and at-home modes, with results typically in about six days against about twelve for the paper test. Near the December cut-off that difference decides whether a retake is possible before the last scheduled date.
