PLAB 2 in Manchester: 16 Scenarios, Eight Minutes Each, What the OSCE Tests

PLAB 2 is the clinical half of the PLAB test: a performance-based assessment of clinical and professional skills, also described as an OSCE, delivered at the GMC's clinical assessment centre in Manchester. The GMC's guide says it is "not intended to test how well you can remember and recite facts"; it tests whether you can do the job of a doctor at Foundation Year 2 level, safely and with patients at the centre. For candidates who have just passed PLAB 1, the format deserves careful attention, because it rewards different preparation.

The format

  • 16 scenarios, each lasting eight minutes, reflecting real settings such as a mock consultation or an acute ward.
  • Around three hours in total, with at least two rest stations built in.
  • A timer announces when to move between scenarios.
  • Standard and references: F2 level; current UK best practice and published evidence; drug names from the most recent British National Formulary.
  • MLA compliance: like PLAB 1, PLAB 2 is compliant with the Medical Licensing Assessment requirements, so its stations are built on the MLA content map.

What a scenario can ask of you

The GMC says the exam tests clinical knowledge, skills and behaviours. In practice, a scenario may be a focused history, an examination on a simulated patient or manikin, an explanation or counselling task, a discussion of management, a practical procedure, or a professional dilemma, usually with a simulated patient and sometimes with a colleague role. Communication is assessed throughout, not in a separate station: how you open, how you check understanding, how you safety-net and how you close are marked alongside the clinical content.

Preparing after PLAB 1

  1. Convert knowledge into eight-minute structures. For each common presentation, build a repeatable approach: introduction and consent, focused questions, red flags, examination if required, explanation in plain language, management and safety-net, summary. Rehearse it against a clock until eight minutes feels long rather than short.
  2. Practise with a partner, not a mirror. Simulated patients respond to what you say; a study partner playing the role, with a mark scheme, is the closest rehearsal available at home.
  3. Learn UK consultation norms. Shared decision-making, ICE (ideas, concerns, expectations), explicit consent for examination, chaperones, and documentation habits are assessed behaviours, not optional extras.
  4. Cover professionalism scenarios. Breaking bad news, a colleague who appears unwell or impaired, a medication error, a capacity assessment, and a patient who refuses treatment all fit the content map's themes.
  5. Plan the trip. The exam is in Manchester; factor in visa, travel and acclimatisation, and book well ahead, because dates fill.

Eligibility and timing

You can book PLAB 2 only after passing PLAB 1, and the GMC's guide links to separate chapters on when and where to take it and on the rules for additional attempts. Check those pages for the current time limits between PLAB 1 and PLAB 2 and for the validity of a PLAB pass toward registration; both are rules that change, and the GMC's own pages are the only authority.

What this means for you

PLAB 2 is passed by candidates who rehearse structured, timed, patient-centred encounters until they are automatic. Knowledge got you through PLAB 1; behaviour under a clock gets you through PLAB 2. FAME's PLAB resources include scenario frameworks and mark-scheme-style checklists for partner practice.

Two questions candidates ask

Can PLAB 2 be taken outside the UK? No. The GMC delivers PLAB 2 at its clinical assessment centre in Manchester; the overseas venues apply to PLAB 1 only. Budget for travel and accommodation as part of the exam.

What visa do I need? Most candidates travel on a UK visitor visa for the examination; the UK government's visa pages set the current rules and processing times, which vary. Apply as soon as the PLAB 2 date is confirmed, because an appointment is of no use without entry clearance.
