SMLE, DHA, DOH or MOHAP: Choosing a Gulf Licensing Exam as a Pakistani Doctor

Pakistani doctors looking to the Gulf meet four licensing bodies: the Saudi Commission for Health Specialties (SMLE), the Dubai Health Authority (DHA), the Department of Health Abu Dhabi (DOH, formerly HAAD) and the UAE Ministry of Health and Prevention (MOHAP). Each licenses for its own jurisdiction and runs its own assessment. Preparing for the wrong one is a common and expensive mistake. This comparison is organised around the questions that decide the choice.
Who licenses where
- SMLE / SCFHS: the Kingdom of Saudi Arabia.
- DHA: the Emirate of Dubai.
- DOH: the Emirate of Abu Dhabi.
- MOHAP: the Northern Emirates (Sharjah, Ajman, Umm Al Quwain, Ras Al Khaimah, Fujairah) and federal facilities.
Within the UAE, the unified Professional Qualification Requirements align titles and eligibility across the three regulators, but each still licenses separately. Saudi Arabia classifies and registers through SCFHS under its own framework.
What they share
- Primary-source verification of degree, registration and experience through the DataFlow process, which takes weeks and should start first.
- A computer-based clinical assessment at the level of the title (general practitioner or specialist), delivered through a testing partner (Prometric for SCFHS, DHA and MOHAP; DOH's provider should be confirmed on its portal).
- Activation by an employer: an eligibility or classification outcome is not a licence until a licensed facility sponsors you.
- Broad clinical content: medicine, surgery, paediatrics, obstetrics and gynaecology and emergency presentations, which is why one preparation serves all four with modest adjustment.
Where they differ
- Exam length and structure: the SMLE is a 4-hour-30-minute single session under Prometric's SCFHS rules; the UAE assessments are shorter and differ by regulator and title. Third-party figures conflict; use the regulators' candidate guidance.
- Classification: SCFHS classifies the professional level after the exam; the UAE regulators license against the PQR title applied for.
- Posts and pay: vary by country and emirate and are outside the scope of an exam article.
How to choose
- Follow the job. The regulator is determined by where the facility is. Candidates with an offer prepare for that regulator's assessment; candidates without one should pick the market they are targeting and the title the PQR or SCFHS gives them.
- Match the title to your qualifications. MBBS with house job and PM&DC registration generally means general practitioner; FCPS or an equivalent recognised qualification with the required experience means specialist. The assessment content follows the title.
- Verify once, apply where needed. DataFlow reports are regulator-specific in their issuance, but the underlying documents are the same; keep a clean, consistent set.
- Prepare broadly, then adjust. The clinical core is common. Add the regulator's blueprint emphasis and the exam's timing in the final month.
What this means for you
Decide the regulator before the question bank, not after. FAME offers separate SMLE, DHA, HAAD (DOH) and MOH question banks precisely because the assessments differ in blueprint and timing while sharing the clinical core.
Quick reference
- SMLE (SCFHS): Saudi Arabia; Prometric; 4 hours 30 minutes; classification after the exam.
- DHA: Dubai; Sheryan portal; Prometric assessment; eligibility letter, then activation by a Dubai facility.
- DOH (formerly HAAD): Abu Dhabi; TAMM services; assessment provider to confirm on the DOH portal; activation by an Abu Dhabi facility.
- MOHAP: the five Northern Emirates; federal licensing system; Prometric assessment; activation by a MOHAP-licensed facility.
- Common to all four: DataFlow primary-source verification, a title tier based on qualifications and experience, a broad clinical MCQ assessment, and an employer-activated licence.
One preparation, adjusted in the final month for the regulator's blueprint and timing, serves all four assessments; the paperwork is what differs, and it is the paperwork that should start first.
