The Saudi Medical Licensing Examination (SMLE) is the Saudi Commission for Health Specialties (SCFHS) examination that physicians must pass to be classified and registered to practise in the Kingdom. It is delivered by Prometric at test centres in Saudi Arabia and abroad, including Pakistan. Prometric's SCFHS page records that, as of 1 March 2023, the duration of the SMLE, the dental SDLE and the pharmacy SPLE was changed to 4 hours 30 minutes. This article sets out what is established about the exam, what you must check on the official portals, and how to prepare for a paper of that length.
What is established
- Owner and purpose: SCFHS sets the exam as part of professional classification and registration of physicians in Saudi Arabia.
- Delivery: computer-based at Prometric centres; eligibility is established with SCFHS before a Prometric booking can be made.
- Duration: 4 hours 30 minutes since March 2023.
- Content: single-best-answer multiple-choice questions on clinical medicine at the level of a graduating physician, across medicine, surgery, paediatrics, obstetrics and gynaecology and the other core disciplines.
What to confirm on the official portals
Third-party sites disagree about the number of questions, the pass mark and the number of testing windows, which is a reason to use the primary sources: the SCFHS portal (Mumaris Plus) for eligibility, exam rules and results, and the Prometric SCFHS page for booking, rescheduling and centre lists. FAME does not publish a question count or pass mark for the SMLE until SCFHS states it in a document we can cite.
The booking sequence
- SCFHS account and eligibility: credentials verification (through the DataFlow primary-source verification process that Gulf regulators use), then an eligibility number for the exam.
- Prometric booking: with the eligibility number, choose a centre and date. Pakistan has Prometric centres in the major cities; dates fill, and SCFHS's rules govern how far ahead booking is allowed and how rescheduling works.
- Exam day: identification matching the registration, arrival in advance, and Prometric's centre rules.
- Result and classification: results are released by SCFHS and feed into the classification decision that determines the level at which you may be registered.
Preparing for a four-and-a-half-hour paper
- Stamina first. The SMLE is a single long session. Practise at least four full-length timed sessions of equivalent duration before the exam; most candidates lose accuracy in the final hour because they have never rehearsed it.
- Breadth over depth. The paper samples the whole of undergraduate clinical medicine; a candidate strong in two specialties and weak in four will struggle. Allocate time by discipline and rotate daily.
- Guideline-based answers. Questions follow standard international management; learn first-line choices and thresholds rather than local habits.
- Error log and recalls. Organise recalled themes by discipline and use them to direct question practice, not as a substitute for it.
What this means for you
Prepare for the SMLE as a stamina test of broad clinical medicine, and run the administrative track (DataFlow verification, SCFHS eligibility, Prometric booking) in parallel from the start, because it takes weeks. FAME's SMLE question bank is organised by discipline with full-length timed sessions sized to the exam's duration.
Two questions candidates ask
How many questions are on the SMLE? Third-party sites quote different figures, and SCFHS's own candidate documents are the only reliable source. FAME quotes the duration because Prometric publishes it, and leaves the question count to the commission's eligibility pack, which you receive once your application is accepted.
Can I sit it in Pakistan? Prometric operates centres in Pakistan's major cities and SCFHS exams are offered internationally; confirm the current centre list and availability on Prometric's SCFHS page when you book, because centres and dates change.
