USMLE Step 2 CK Study Plan for Pakistani Graduates During House Job

Step 2 CK is the examination with the score, now that Step 1 is pass/fail, and most Pakistani candidates sit it during or shortly after house job. That is the right time: the wards teach exactly what Step 2 CK tests, if you let them. This plan runs six months and assumes a working house officer with two hours on most days and longer sessions on days off. It is built around one rule: the rotation you are on is the system you study.

The examination, briefly

Step 2 CK has 316 multiple-choice questions over a nine-hour day, delivered since May 2026 as 16 blocks of 18–20 questions, 30 minutes each. You can review within a block but not return to a closed one. The content is clinical: diagnosis, next best step, management and prognosis across medicine, surgery, paediatrics, obstetrics and gynaecology, psychiatry and preventive medicine, with heavy use of guideline-based thresholds.

Months 1–4: study by rotation

  • On a medicine rotation, work through the medicine question sets; on surgery, the surgical ones; and so on. What you see on the ward in the morning you answer questions about in the evening, which is the most efficient learning available to a house officer.
  • Daily target: 40 questions timed in two 30-minute blocks, with every explanation read. On post-call days, one block.
  • Days off: a 100-question mixed session, timed, followed by analysis.
  • Keep a running document of US-specific differences from Pakistani practice: screening ages, vaccination schedules, first-line drugs, and the management thresholds that recur in questions.

Month 5: fill the gaps

Sit the first NBME self-assessment at the start of the month; it runs on the same software as the real exam, so it also rehearses the interface. Use its breakdown to find the two or three weakest systems and spend the month on them, keeping the daily question target. Add the areas house job under-serves: psychiatry, ethics and communication, biostatistics and preventive medicine.

Month 6: simulate and taper

  • Two full-day simulations in the first three weeks: 16 blocks of 30 minutes with planned breaks. Practise a steady 90 seconds a question and a fixed rule for flagged items.
  • A second self-assessment in week two to confirm the trajectory; book the real exam when the practice scores sit where you want them, not before.
  • Final week: error log, high-yield thresholds, sleep. No new resources.

Administration you must not leave late

  1. ECFMG certification eligibility must be in place before FSMB can register you; since January 2026 FSMB handles USMLE registration, scheduling permits and score reports for IMGs.
  2. Choose an eligibility period that lets you test when ready, and book the Prometric seat early; Pakistani centres fill in peak months.
  3. If you are aiming at the 2027 Match, remember the Pathways requirements run in parallel, including OET Medicine by December 2026.

What this means for you

House job and Step 2 CK are allies, not competitors, when the study follows the rotation. Six months of timed questions, two self-assessments and two full simulations produce a score you choose, rather than one that happens to you. FAME's USMLE Step 2 plan organises the question bank by system to match rotations, with 30-minute block timing built in.

Two questions house officers ask

Can I sit Step 2 CK before Step 1? The USMLE programme allows Step 1 and Step 2 CK to be taken in either order; Step 3 requires both. Most Pakistani candidates still take Step 1 first because its basic sciences underpin Step 2 CK reasoning.

How many questions does this plan add up to? Roughly 40 a day for four months and 50 a day for two, plus the weekend sessions, self-assessments and two full simulations: about 8,000 questions, which is the volume candidates who score well typically report.
