USMLE Step 3 Passing Standard Under Review: Decision Due November 2026

On 2 September 2026 the USMLE programme announced that its Management Committee will review the passing standard for Step 3 at its November 2026 meeting. If the committee decides to change the standard, the new one will apply to every examinee who takes Step 3 on or after 1 January 2027. For IMGs who are in or about to start US residency, the announcement raises a simple question: should you test before the new year?

How a passing standard review works

The programme reviews each Step's passing standard every three to four years as part of its normal procedures; this is a scheduled review, not a response to any incident. Independent panels of physicians and educators met in September 2026 in standard-setting exercises. Their recommendations go to the Management Committee alongside surveys of licensing representatives, residency programme directors, faculty and examinees, trends in examinee performance, and analyses of score precision and its effect on pass/fail decisions. The committee then decides whether the standard should move.

What is known and what is not

  • Known: the review date (November 2026), the effective date of any change (1 January 2027), and the inputs the committee considers.
  • Not known: whether the standard will change at all, and if so in which direction. Past reviews of other Steps have sometimes raised the standard and sometimes left it alone. The programme has said the final decision will be posted on its website.

Anyone telling you the new Step 3 pass mark is already fixed is guessing.

Why the timing matters for IMGs

Step 3 is taken by many IMGs during the first year of residency, and some sit it before the Match to strengthen an application or for visa reasons. A candidate who tests in December 2026 is assessed under the current standard; one who tests in January 2027 is assessed under whatever the committee decides. If the standard rises, the December examinee has the easier threshold; if it stays, nothing is lost by testing earlier, provided the preparation is complete.

What this means for you

  1. If you are ready and can schedule before 31 December 2026, do so. There is no downside to the current standard, and no upside to waiting for a decision that may raise it.
  2. If you are not ready, do not rush. A fail under the old standard is worse than a pass under a new one. Step 3 is a two-day examination with multiple-choice blocks and computer-based case simulations; it rewards thorough preparation, not calendar games.
  3. Remember the software change. Step 3 has used the programme's new test-delivery software since 10 March 2026, with the updated interface, keyboard navigation and image-contrast settings. Use the current tutorial, not an older one.
  4. Check eligibility and scheduling through FSMB. Since January 2026, FSMB manages all USMLE services for IMGs across the three Steps; ECFMG continues to determine certification eligibility.

The bigger picture

This review sits alongside two other USMLE changes: the May 2026 move to shorter, more numerous blocks for Step 1 and Step 2 CK, and the planned transition to designated testing dates in January 2029. None of the three changes alters what the exams test; together they change how and when they are taken. FAME's USMLE section will post the committee's November decision as soon as the programme publishes it.

Two questions candidates ask

If I test in December 2026, could my result be re-scored under a new standard? No. The programme states that a new standard would apply to examinees who take Step 3 on or after 1 January 2027; a December examinee is assessed under the standard in force on the test date.

Where will the decision appear? On the USMLE website, which the announcement names as the place for updates and the final decision. FAME will link to it from this article when it is published; until then, no number is official.
